Identify who is asking, sending and receiving
Begin with the receiving practice's actual request, then clarify the arrangements with the current practice. Record who should answer a question and what that practice has confirmed. Avoid replacing that exchange with an assumed procedure from a website or another person's experience.
SMILE-FX's published transfer outline mentions a signed release and prior scans, X-rays and treatment notes. It also describes a new examination and review of progress and goals. Ask the practice to confirm the requirements and arrangements for your situation.

A fictional example: Maya follows the acknowledgments
Maya has discussed a records request with both practices. She first writes 'handoff done,' then notices that her note does not say what was actually acknowledged. She replaces that label with the more specific sequence below.
Each row preserves a separate question. The receiving practice's acknowledgment of receipt would answer a delivery question; it would not fill in an unconfirmed acceptance or appointment row.
| Topic in the fictional exchange | What Maya can record | Who should clarify | Open question |
|---|---|---|---|
| Material requested | A request was discussed; exact requirements need confirmation. | Prospective receiving practice | What existing material is needed for its assessment? |
| Sending arrangements | Maya has asked about the process; sending is not confirmed. | Current practice and receiving practice, for their respective arrangements | What method and authorization arrangements apply? |
| Receipt | No acknowledgment is recorded. | Receiving practice | Has the requested material arrived? |
| Assessment and acceptance | Not confirmed in the note. | Receiving practice | What review or conversation is needed, and what has been agreed? |
| Appointment and care arrangements | Separate confirmation still needed. | The practices responsible for the arrangements being discussed | Which dates and contact responsibilities have actually been confirmed? |
Keep an acknowledgment note, not a blanket 'complete' label
A useful entry identifies the topic, the practice or contact involved, what was actually acknowledged and the date of that acknowledgment. Add a next question when a step is still open. If you have only your recollection, label it as such until you can clarify it.
For example, Maya could record that the receiving practice confirmed receipt on a particular date while leaving assessment and appointment status unconfirmed. That keeps a delivery update from becoming a care agreement. Update only the part the new information actually answers.
- Topic or arrangement being discussed.
- Practice to clarify it and the contact route it has provided.
- Acknowledgment actually received, with date or reference.
- Current status in plain words, including unknown or pending.
- Next question and who should answer it.
Confirm the records route with the practices
Ask about the actual delivery method, formats, authorization arrangements, possible fees and timing. This guide does not provide a release form or decide who may sign one. Do not send records through an unconfirmed channel or use the consultation request as a records-delivery route.
Financial arrangements also need their own confirmation. A statement from one practice does not establish the terms with the other. Ask each practice about its applicable agreement; do not infer a credit, refund or transfer price from the fact that records are being discussed.
