Prepare the conversations between you and the practices
Start with your existing arrangements. Then separate the information the practices need to exchange from the appointment and contact details you need to confirm. You can enter any chapter directly; the aim is a usable set of notes, not completing a form or proving that a transfer must happen.
| Preparation job | Useful output |
|---|---|
| Current-treatment summary | A dated snapshot that distinguishes practice-supplied information, your recollection and move logistics. |
| Records and practice handoff | A note showing what was requested, what was acknowledged and who should clarify the next step. |
| Appointments and care questions | Separate rows for confirmed dates, pending requests and contact arrangements to discuss. |
A move starts practical questions, not an automatic handover
You may know where you will live before you know which care arrangements are possible. Record that difference. A practice receiving information is not the same as confirming an appointment or agreeing to take over care. Ask what each confirmation means rather than filling in the next step yourself.
Relocating does not mean your earlier care failed or that you have already decided to switch. Keep your own priorities in the discussion and use your treating team's instructions for current concerns.
Considering the SMILE-FX studio in Miramar
SMILE-FX publishes information about accepting transfers with braces already on and reviewing current progress. Individual acceptance and arrangements still need confirmation. This practice-owned guide is for people in Broward and nearby Miami-Dade considering its Miramar studio.
