A requested date is not a confirmed appointment
Keep an existing appointment, a requested assessment and any later arrangement in separate rows. Include the practice and confirmation reference where you have one. A move date can explain a practical constraint without setting the date or spacing of clinical care.
If travel could affect an existing arrangement, ask the relevant practice what needs to be discussed. This guide does not determine a safe interval, tell you to cancel an appointment or set a date when responsibility changes. Those arrangements need an individual conversation with the practices.
A fictional example: Maya checks what each entry means
Maya's calendar contains a moving day, an existing appointment and a preferred date mentioned in an inquiry. She initially puts all three under 'next steps.' She then notices that only the existing appointment has a practice confirmation. The preferred date is still a request.
Maya separates the entries so that a personal plan does not appear to confirm a receiving appointment. She can bring the unresolved points into the practice conversations without inventing a new care timetable.
| Fictional entry | Date or status | Contact or practice to clarify | Question still open |
|---|---|---|---|
| Moving day | Date in Maya's personal plans | Maya can explain the practical circumstances to the practices | What existing arrangements could be affected? |
| Existing appointment | Date in the current practice's confirmation | Current practice | Does the move require a discussion about this appointment? |
| Requested receiving assessment | Preferred date mentioned; no appointment confirmed | Prospective receiving practice | What date and assessment arrangements, if any, can it confirm? |
| Later care arrangements | Not confirmed | Practices involved in the proposed handoff | What has actually been agreed about subsequent care and contacts? |
Record contact instructions as carefully as dates
Use your treating team's contact instructions for a current concern. Keep the route for asking about a prospective assessment separate. An inquiry to a new practice does not establish that it has accepted the case or agreed to manage a current appliance problem.
Ask the practices which contact arrangements apply while the proposed transfer is being discussed and what will need confirmation if the arrangements change. Do not assume that a records acknowledgment, a consultation request or a calendar entry has answered that question.
- Which contact instructions should I use for current treatment questions?
- Who should answer a question about the requested assessment?
- What appointments or other arrangements have actually been confirmed?
- Which care and contact arrangements still require a conversation?
A printable confirmation summary
For each event or request, note the date only if it is known, the source of confirmation, the practice to ask and the unresolved question. Date your own update. Words such as 'requested,' 'confirmed by the practice' and 'not yet clarified' are more useful than an unexplained checkmark.
When Maya receives new information, she changes the relevant row and keeps other pending items visible. The summary helps her prepare the next conversation; it does not authorize treatment changes or declare a handover complete.
