# Start with a dated picture of the current arrangements

Preparation chapter 1

A short summary can help you explain the situation without turning your recollection into a clinical report. Attribute information to its source, keep move logistics separate and leave unanswered items visibly open.

## Separate what the practice supplied from what you remember

The current practice may have supplied a written description, a progress summary or appointment instructions. Refer to that material as supplied; do not rewrite it as your own assessment of how treatment is progressing. Your personal notes can say what you remember and which point you want checked.

Move details belong in another part of the note. A travel date or new address explains a practical circumstance. It does not determine a treatment interval, confirm a receiving appointment or establish who will provide care. Keeping those categories apart makes the next question more specific.

## A fictional example: Maya separates the facts in her note

Maya is wearing braces and preparing to move. Her first note says, 'Moving soon; need the next appointment at the new practice.' It combines a known practical event with an arrangement that has not been confirmed.

Maya dates a new snapshot and separates the information below. The move is still a practical planning matter, and a receiving appointment remains a question to resolve with the practices.

| Part of Maya's fictional snapshot | What is recorded | What remains to clarify |
| --- | --- | --- |
| Snapshot date | Fictional update made on 26 October 2026. | Update the note when a practice confirms something new. |
| Move circumstances | Maya has a moving date in her personal plans. | Which practical arrangements could that affect? |
| Current practice's information | Reference to the written information Maya already has. | Does the current team need to clarify or supply a summary? |
| Maya's recollection | Maya remembers a conversation about a later visit. | What did the team actually confirm? |
| Receiving practice request | Maya has asked about an assessment. | Is the case accepted, and has any appointment been confirmed? |

## A printable snapshot for your own use

Use these headings on paper or a printed copy. A reference can point you back to a message or document without copying health information into this website. You choose what you want help organizing; the guide does not receive your notes.

- Date the snapshot was made or updated.
- Move or travel circumstances you want to discuss.
- Current practice's wording or document reference, kept distinct from your recollection.
- Appointments actually confirmed, with the confirmation reference.
- The treating team's contact\-instruction reference.
- Open questions, and the practice you need to ask.

## Use the snapshot to identify the next conversation

Maya can now ask the current practice to clarify what it has confirmed and ask the prospective receiving practice about its assessment arrangements. She does not need to call a remembered plan an appointment or label a request as an accepted transfer.

If someone helps with notes, you can choose the role you want them to have. Helping organize the snapshot does not establish authority to release records or speak for you; confirm any necessary arrangements with the practices.

## Your individual next step

Ask about care in Miramar during your move.

[Request an in-person consultation](https://www.smile-fx.com/lp/free-consult/?utm_source=braces-transfer-broward&utm_medium=microsite&utm_campaign=smile-fx-guides)

This opens the SMILE\-FX consultation request page; it does not submit a request. The practice must confirm transfer acceptance, assessment scope and fees, timing and record arrangements.

Preparation for an active\-braces move. Use the notes for your practice conversations; individual care arrangements need confirmation.

For a current treatment concern, use your treating team's contact instructions. A consultation inquiry does not establish a transfer, a change of care responsibility or urgent support.
